- Your employment history including:

- Name of company
- City and State that the company is located in.
- Inclusive dates of employment. Include month and year.
- Description of what you did on the job and what you can do as
a result of the employment.
- Education:
- Name of Program
- Name of School
- City and State where located
- Date started (month and Year) to Present or last date attended.
- Graduated (Yes/No) What you learned and/or what you can do as
a result of your education.
- Other applicable Information if it applies to the type of
