Confidentiality
Every student who is treated at Student Health Services
has a confidential medical record. If you decide that this information
may be released, you must sign a written consent form; the only exceptions
are where the law requires medical information for reasons of public health
and safety.
If you are under the age of 18, it will be necessary for your parent or guardian to complete a medical treatment authorization form in order for Student Health Services to provide medical treatment.
