Instructions for Completing Internship Contract

Call the service-learning coordinator, (231) 995-1294, to discuss possible internship placements, learning objectives, etc.

Course Code Number (upper left corner) — will be issued and added by the coordinator after the completed contract has been approved.

Catalog Number — department letters (PSY, MKT, ENG, etc.) 290. Be sure to select a prefix that you want.

Title — Academic Service Learning Internship (already provided).

Credits — may be 1-4 per semester or session. (a minimum of 32 service hours per credit is required; 40 hours for the Social Work Program.)

Hour/Week, Beginning and Completion Dates — should reflect the hours needed for the credits earned.

Questions 1 and 2 should be answered in consultation with your field supervisor; question 3 should be filled out prior to meeting with field supervisor; and question 4 with your instructor.

A REFLECTIVE JOURNAL (see "Tips") IS REQUIRED AS PART OF THE FINAL GRADE.
