Disabilities and Documentation

Disability Documentation
Hearing Loss Physician's Statement of Long Term Hearing Loss or Speech and Hearing Evaluation
Visual Loss Physician's Statement of Long Term Visual Loss or Commission for the Blind Evaluation
Physical/Mobility Impairment Head Injuries A physician's statement indicating the condition as a long-term disability rather than a short-term disability
Specific Learning Disability Psycho-Educational Report or Multidisciplinary Evaluation Team (MET) Report: Intelligence Test, i.e.: WAIS or WISCIIIAcademic test, i.e., KTEA, WIAT, or Woodcock-Johnson Battery (administered within three years by a Certified School Psychologist, Licensed Psychologist, or others qualified to administer the tests)
Attention Deficit Disorder or Attention Deficit Hyperactivity Disorder Physician's Statement of Long Term Disability or Comprehensive evaluation report, which includes: a diagnosis, AD/HD behavior rating or evaluation outcomes, and accommodations. (Administered within three years by a Physician. Information can be acquired by a Certified School Psychologist, Licensed Psychologist, or others qualified to administer the tests)
Emotional Impairment Statement of Long Term Disability or Multidisciplinary Evaluation Team (MET) Report administered within three years by a Certified School Psychologist, Licensed Psychologist, Social Worker, or others qualified to administer the tests)
Psychiatric Disabilities Physician's Statement of Long Term Disability or Psychiatric Evaluation administered within three years by a Licensed Psychiatrist, Licensed Psychologist, or others qualified to administer the tests)
  • All documentation must be within the last three years.
  • All documentation must be on file before services are provided.
  • All students must be registered for classes before services are provided.
